Position Overview

Inabia is seeking a compassionate and motivated Physical Therapist to provide rehabilitation therapy services to residents within a senior living and skilled nursing community. The therapist will help residents maintain or regain mobility, independence, and overall physical health.

Key Responsibilities

• Conduct comprehensive physical therapy evaluations to determine patient mobility limitations and rehabilitation needs.
• Develop individualized treatment plans to improve strength, flexibility, balance, and coordination.
• Provide therapeutic interventions including mobility training, strength exercises, manual therapy, and balance rehabilitation.
• Assist residents recovering from surgery, illness, stroke, or chronic conditions.
• Monitor therapy progress and update treatment programs based on patient performance and goals.
• Educate residents and caregivers on safe mobility practices, fall prevention, and home exercise programs.
• Maintain accurate therapy documentation including evaluations, treatment notes, and discharge summaries.
• Work collaboratively with interdisciplinary care teams including nursing staff, physicians, occupational therapists, and speech therapists.
• Participate in care planning meetings and support safe discharge planning.

• Requirements

• Active Physical Therapist License
• Degree from accredited Physical Therapy program
• Experience in rehabilitation or senior living environment preferred
