Job Summary:

The Licensed Practical Nurse (LPN) in a Skilled Nursing Facility (SNF) is responsible for providing direct patient care under the supervision of an RN or physician. The LPN ensures quality care for residents, including medication administration, monitoring health conditions, and supporting daily living activities.

Key Responsibilities:

• Administer medications and treatments as prescribed
• Monitor and record patient vital signs and health status
• Provide basic nursing care including wound care and dressing changes
• Assist residents with daily activities (ADLs) such as bathing, dressing, and feeding
• Collaborate with RNs, physicians, and healthcare team members
• Maintain accurate and timely patient documentation
• Respond to patient needs and emergencies promptly
• Ensure compliance with facility policies and healthcare regulations
